TELEHEALTH INFORMED CONSENT

1. Nature Of Telehealth

Telehealth involves the use of electronic communications to enable a licensed, independent U.S. healthcare provider to evaluate you and, where appropriate, prescribe medication without an in-person visit. By using 1776 Health, you consent to receive care through telehealth, including the review of the medical intake information you provide and a consultation conducted by phone.

2. Who Provides Your Care

1776 Health is a technology platform. The clinical evaluation and any prescription are provided by a licensed U.S. provider through OSI Medical Services (authorized in all 50 states), who may be a physician, nurse practitioner (NP), or physician assistant (PA) depending on your state and circumstances. Your medication, if prescribed, is dispensed by Pharmacy Hub LLC, a U.S. compounding pharmacy licensed in all 50 states. Using this platform does not create a provider-patient relationship with 1776 Health itself.

3. No Guarantee Of A Prescription

Completing an intake and paying for a treatment does not guarantee that you will be prescribed medication. The provider will determine whether treatment is appropriate for you based on your medical history. Not all visits result in a prescription. If treatment is not approved, you are refunded for the medication.

4. Benefits And Risks Of Telehealth

Benefits include improved access to care and convenience. Risks and limitations include, but are not limited to: the provider's evaluation is based on the information you provide and cannot include a hands-on physical examination; technical problems may affect the consultation; and in rare cases information transmitted may not be sufficient to allow appropriate clinical decision-making, in which case the provider may decline to prescribe or may recommend in-person care.

5. Controlled Substances And Scope Limits

1776 Health does not prescribe controlled substances through this platform. Services are available only to U.S. residents who are 18 years of age or older. The compounded medications offered are not FDA-approved as combination products.

6. Consent To Electronic Communications

You consent to receive communications from 1776 Health, the provider, and the pharmacy electronically, including by email, text message, and phone, regarding your care, your order, and your account. You may be contacted by phone for your consultation and for follow-up.

7. Emergencies

Telehealth is not for emergencies. Do not use this service for medical emergencies. If you are experiencing chest pain, sudden or serious pain, bleeding, or other serious complications, call 911 or go to the nearest emergency room.

8. Your Rights

You have the right to withhold or withdraw consent to telehealth at any time without affecting your right to future care. You have the right to ask questions about your treatment and to request your medical records. Your health information is protected under our HIPAA Notice of Privacy Practices.

9. Acknowledgment

By completing your medical intake and continuing with 1776 Health, you acknowledge that you have read and understood this Telehealth Informed Consent, that you consent to receive care via telehealth, and that you have had the opportunity to ask questions by calling 1-877-791-1767 (Mon–Fri 9 a.m.–4 p.m. CST).
